what does my bad mean and why do so many people say it? i find it truly annoying and hear it all the time on the tv

As they said: My fault, but, it can also mean "My mistake." Example: You walk up to someone, thinking they're someone else, and you say your friends name. They turn around and give you that look, or be like "I'm not that person" you'd say "Oh, my bad, sorry" then continue on your way.
That's how it works from what I've seen.
