"Pockets full of Posies"
Posies are a flower, I believe, was said to help cure the unknown disease.
"Ashes, Ashes"
They had to burn the bodies, to try and stop the spread of the disease.
"They all fall down"
...Means that they all died off.
And thats the story of the song. ;D
