Well the way I learned was this tutorial:

But basically what you do is open Photoshop---> File---> New---> Width: 350 px Height: 19 px---> Use Gradient Tool to put on your two colors---> Use the Pattern Stamp Tool for lines---> Place your Image---> Remove any access/unwanted parts of the image using the Magic Wand Tool---> Use the Visitor Font to place text---> Layer---> Layer Style---> Stroke---> Choose the color stroke you want on your text & the px size---> Add an eclipse at top with the Eclipse Tool---> Change the fill to about 25%---> Add a border by selecting the whole image with the Lasso Tool---> Edit---> Stroke---> Choose the border size and color

But the tutorial has screen grabs that make it easy.

To place the image in Photoshop its File -> Place. Then you have to go through your files to find the picture you want to put on. To get the Gradient Tool, right click on the Paint Bucket and you should see a little sidebar come out of the box with the picture of a square on it. Click the square and thats the Gradient.
